I haven't been in a situation where I've had to wait in some time for a while now. When I have in the past it gives me the "I'm being set up" vibe and I usually cut and run. My senses say it's a sting and they're not quite ready with all the "players" in place. Paranoid is my middle name. The last time this happened to me I told the provider I was leaving if she didn't send me a photo of her boobs right away. She complied and everything worked out. My time is worth as much as theirs is, probably more if I'm being honest so in any case I'd wait for 10-15 minutes then I'm out.

No way would I ever wait 45 minutes. Always have a backup plan. That don't mean set two Appointments just contact another provider ask if they will be available at the time you set with the first. If it falls through go to number 2.

Typically I will wait 15-20 minutes as I think that's reasonable and I want her to freshen up first. Often when they need you to wait it's because they've double booked and took the first one to get there. If I have time I wait but if not I split.
