I've come to know that the topic of STDs and STD treatment are stigmas in Japan, thus contradicting the cleanliness argument. Cleanliness does not imply STD awareness. It's about managing risks. Almost all of the permanent ones now either have very effective PrEP or have vaccines. Herpes does not, and Herpes can spread when not in an outbreak, but the risk of getting it from someone not having an outbreak is relatively low. A lot of the curable ones have post exposure prophylaxis to reduce the risk of getting a full-on infection.
