At many places you don't have to ask at all -- they ask you. Typically after the flip, although rarely before you even get started. But yes, sometimes at the more legit places, you may have to bring up the subject. Although if there is no hint of extras in advance (e.g. towel completely removed), I generally don't ask, and just consider it a legit place.
