For independents you might try Luna in Mira Mesa / Scripps, she has most of those elements and you may just need to work it out with her. I would say more than 90% of providers who advertise on RR that are 'established' (reviewed) charge 200 $+ for the hour and extras are more. Probably making a list of <200 $ would make more sense.
