That's not surgery per se. That's covering up the bruising / scabbing / abscesses caused by injecting into the femoral vein. Most of their usual injection sites have constricted because dope is cooked with so much filler garbage and they can't hit there anymore. After that, they usually get cellulitis and, worse yet, blood clots.

They might get "surgery" to remove an abscess or a deeper cellulitis infection.

I have a lengthy post about the best ways to get intel. But it's not for the faint of heart.
