USA Barry you make some great points. For me it's more about enthusiasm and attitude more so than age. It's a real turn on to me to have a massage girl excited (or seem to be excited) to see me as long as she is clean and has a good personality.

Many CAMPs give 4-hands. I get them all the time. Typically I ask for a 4-hand from the start, but if I only have one masseuse and her HJ skills aren't up to par, I'll ask for a second girl to help out. Not all CAMPS will do that though, even CAMPs where extras are the norm.
