I do not understand what is offensive there, If I am paying my hard earn money I need some satisfaction or at least some interest in return. If you are not interested say it in advance and I will not come see you. I don't think he was implying that he was trying to hurt her. I think he was just saying that because he put down his hard earned money and she lumped on conditions (one of which-getting on top-was IMO unreasonable) he did not hold back in terms of sexual aggression. I'm sure she made no attempts to stop the session and looks of resignation (the chick does not want to be there but wants the money and is not sexually aroused therefore is dry and a bit uncomfortable) can often be misinterpreted as pain. As far as the blood goes, I've had a provider start her period during a session before and she did not offer a full or partial refund or to pay for the girl she was travelling to switch with her.
