I have done it twice. One provider did it for free with no upcharge. I asked beforehand and she said she liked doing it and didn't charge for it. Another wanted $40 and settled for $20 without much resistance. She also very much enjoyed the act. I have seen as mmuch as a $60 upcharge for it. So I would say $0. $60 is the range, and it depends on whether the provider really wants to do it and enjoys it. Is it all that it is hyped up to be? Hard to say.
