There's a lady touring my area who I wanted to see. She had a similar form on her website that asked for all sorts of crazy shit; full name, home address, employment info, position at work, an "official story" to tell whomever answers the phone when she calls you at work etc. I bypassed the entire form and simply emailed her respectfully requesting an appointment while explaining my privacy concerns.
