It's different for different people. I would hope that the mongers doing it are either doing it because they're just curious what it would be like, or because they actually have some sort of connection with each other. The extra money would be to guarantee the sex, I would imagine. If I was paying the extra money, it would be because I wanted to guarantee multiple rounds of sex over the course of however many hours.
