That's a symptom of HSV2. NO ONE in their right mind is knowingly seeing you; condoms are not guaranteed to prevent the spread of HSV2.

Every provider listed here I've seen. My older username is one of the first people to post on this forum. I could tell you the exact woman who I saw before I got the bumps (not the big one, I was tested already and already on antibiotics.)

Rather than worrying about if a woman will see you, perhaps you may want to see a doctor first to see what you have!. Don't risk your health, ours or hers if you have something.

You asked if there are good doctors and hospitals in Panama. Punta Pacifica (affiliated with Johns Hopkins) in Panama City, but I wouldn't want to live in Panama City. My choice would be Boquete (an American area), but I have no clue about doctors / hospitals there.
