Thanks to Genghis Dong for the good, organized list he put up. For the places he listed, the rate is the rate. If you only want a HJ and bodyrub, they'll do that for you, but you have to pay the same fee, so therefore nobody does it. Best bet for "non-GFE" is another set of places, most of which actually have a mix of GFE and non-GFE providers. I made a list trying to follow Genghis Dong's format.
