Question #1: Arm Placement. How do you keep your arms when you are laying face down? Are they down, straight by your sides or are they up and folded up by your head?

Personally for me the arm placement just depends on how I am feeling that day and I believe at least once I had my arms folded up by my head and the girl directed them to my sides. I usually do not start touching the legs / butt area until she has a firm grasp on my nuts.

I put mine on my side and chest. I rarely touch as it's distracting. I put my hands behind my head once and she pulled it down. I believe it's rude for them, showing that you believe you're the king and that she's a servant jacking you off.

This may sound funny. But, yes, these is such method and this method is what it sounds, jackhammer your dick where they use a vice of a grip and just pound up and down. My guess is it wasn't voluntary. Pictures and phone number is still posted on the windows. If it was sold to another person they would still be open. If they closed the place voluntarily I would think the landlord would have cleaned up the windows and put a for rent sign up.
