The answer is: it's complicated. The two types of herpes simplex viruses in humans are HSV-1 and HSV-2. HSV-1 is oral herpes. 67% of the population has it. Most people who have it got it when they were children by sharing utensils / drinks / etc. With their parents who had it. It's spread very easily through non-sexual contact. If you get cold sores on your lips, that's HSV-1.

HSV-2 is genital herpes. It's spread through sexual contact and can be spread even when no outbreak is occurring. The estimate is that 13% of the population has HSV-2 However, more than 80% of the people with HSV-2 have no idea they have it. Blood antibody tests are extremely inaccurate. You could test positive for HSV-2 through a blood test one week and test negative the next week. So it's safe to assume that the percentage of the population with HSV-2 is much higher than 13%. Also, the CDC reports that 48% of Black women in the US have HSV-2. The prevalence of the virus among all women is estimated to be 21% and among men 11%. The reason for this is because women have a larger mucous membrane in their genital area than men do and also because women are more likely to be tested for it at their annual gyno exam. Men don't see a urologist every year. The majority of people who have HSV-2 will experience either only one or no outbreaks their entire lives. The only guaranteed way to know you have it is to get the secretion from a lesion tested during an active outbreak.

Here's where it's a bit more complicated. A person with cold sores (HSV-1) can spread it to the genitals of another person during oral sex. So that person can contract HSV-1, but in the genital area. And vice versa, meaning HSV-2 can be spread from the genitals to the mouth during oral sex. Which leaves people with oral-HSV-2 or genital-HSV-1. There's really not much of a difference between the symptoms, since the lesions and blisters will look the same on whatever body part is infected. If you're worried about passing something from one of these sugar babies to a significant other, it's better to contract HSV-1 since that can be spread by non-sexual contact. You can explain it away as you taking a sip of your friend's beer to taste it at the bar and didn't know he had herpes. But HSV-2 is harder to play off because it can only be spread through sexual contact.

So the safest way to go through life is to assume that everyone you have sexual contact with has some form of herpes. But if they pass it to you, you can go your entire life not knowing that you have it because it shows up negative on blood tests and never presents any symptoms. The real kicker is that condoms don't prevent you from contracting HSV-2. You could be having covered sex with a woman who is showing no signs of an outbreak, and you can still end up contracting it if her body is shedding the virus.

Fun fact: Pretty much every porn star has HSV-2. They have to get tested regularly, but the Talent Tests are only for HIV, gonorrhea, syphilis, chlamydia, hepatitis, Trichomonas Vaginalis, and now COVID. They don't test for HSV or HPV. Because HSV-2 is so common, incurable, not life-threatening, and able to be spread with no visible symptoms, and because blood tests are inaccurate, the industry sees no reason to test for it.
