In most cases they are just referring to a physical phone. So you could just go into walmart get a $20 phone and $25/ month card and you would be good. You would call this a burner phone but generally they are referring to burrner app (s). Just like Google Voice, etc.

My personal phone has been prepaid for years like that and haven't had any problems so far.

Some of the providers use a service where they enter in the number that then tells them what the phone provider is. Not sure what other info it tells them but they use that information.
