First of all, every hobbyist has his preference of girls, price range, criteria of what counts as good session, and their level of risk tolerance. Price is a factor that confounds with everything and it's a relative. My golden rule is it's always YMMV and to one of his own.

I agree to Ron's rule about spending more doesn't always guarantee a good session. But the opposite is also true. Spending $150-$200 per hour doesn't always promise you have a great play with the girl who have the appearance that you'd appreciate. Multiple rounds are standard at most agencies, whether it's middle of the road or higher end. I'd say the same thing is true with many middle to high end independents. As with all things, it might vary by the individual and the specific situation.

Never had to negotiate a tip or do "a la cart" pricing, at this level of the scene it's a set amount up front. I don't tip beyond the initial envelope, but do repeat a lot which I've been told multiple times is the best form of tipping.
