Most agencies have flat rates (some can have a tiered rate structure like X for GFE, why for PSE). In this scene I've never encountered someone changing the rate during the visit, asking for extra, or anything like that. I drop the envelope at the beginning somewhere they can see, they may or may not disappear to the bathroom to check it, and after that it's not brought up or thought of again.
