You give. 6 to the mama and when the girl comes in you give her $ and tell her you want full service. They speak some English usually and will understand. When you get to the table shower, let them clean it off first. If they ask if you've been there before, say no but, you've been to other places. Be polite but, go for what you want and you should be alright.
