I don't disagree with the certified whacko portion of your post. She is certifiably crazy. As for extras: I don't push them, I mention the ideal massage and how a true tension release is the only way to go. That alone usually plants the seed, then on the flip, I would say 9 times out of 10, I get the "what else would you like me to massage?"
