I apologize in advance for this question. But, I've been doing the escort thing for years and completely avoiding the strip clubs.

I've gotten extras in strip clubs, but they always came in the context of getting dances. I never paid over and above the cost of the dance. Now, it seems like extras are standard operating procedure. When you guys quote prices for services are these prices above and beyond the cost of the dances so that you're paying a hundred bucks for a blow job plus paying $20 a dance? Or is the hundred bucks and "all in" number that includes any dances that are incidental to the blow job?

It depends on the girl and the club. Always negotiate before going back for dances to get the service you want. But to answer your question, most charge a flat rate if they think you're experienced. If they smell a newbie, you will get charged $25 / song plus you xxxtras fee.

Typically you can say you want a BBBJ and the girl will likely quote you $80-$100. She is assuming you will finish in the typical 3-5 songs. If you request additional services like FS, she is thinking 4-6 songs. If you take longer, most will complain or demand more money.

My first time getting xxxtras I asked for BBBJ, cFS and BBBJTCwCIM. I had disclosed it was my first time, and got charged $20 / song in addition to $150 for the xxxtras. It took about 30 minutes, and she charged me $350 total. The next visit I requested the same thing, but made sure to let the dancer know I was experienced and got the same services for $150 total.

Make sure you negotiate your price / service before you head to the back and that's what you will pay, plus of course the extortion VIP / booth / bouncer fee and tip if she goes above and beyond.
