It is just the law of averages. Chicago proper and surrounding communities has more in the way of population that all of Wisconsin. Thusly there is a higher percentage of people willing to pay whatever rates are proffered. That said, the same can be said for smaller communities. When there is only 1 provider for 50 miles, they can pretty much set their own rates. If you want to pay $150 for a BJ, have at it. You want to pay $300-$500 for a laxidasical visit, good for you. Might I suggest that the reason the pickings are so slim around here is because quality goes where they can make some money. We loose everything decent to Chicago.
