I understand the need to take precautions for a variety of reasons, but to give out this kind of personal information to strangers is ridiculous! I don't know who these people are, if they are going to keep my information on file, use the information to create new identity theft, or even worse, use it against me as blackmail. In either case, I don't care. These establishments are in the entertainment / hospitality industry. They should be grateful that I have visited them or even have considered seeing them. Their is plenty of these shops around Manhattan in the surrounding areas. I have been fortunate to find one or two locations in Flushing / Queens who take good care of me without all this headache.

I'm sorry for the venting, I'm just trying to find out if all or a majority of the establishments in Manhattan follow similar guidelines.
