I have been a member of P411 for a few years now. It's a legitimate service and I've never had any concerns, but it's really only useful if you have the inclination (and budget) to book expensive escorts who insist on invasive screening.

To sign up you have to either screen with P411 themselves, or know 2 girls who are listed on P411 that will give you references.
