Having tried many different approaches, the highest percentage of success for me has now become the only one I use. I wait until we're actually face to face, and I ask if she kisses. I always ask up front. Their response determines if I push the subject. I always ask if we can just skip the cover to start, most will say yes. Once you get to that point in the session, if she's even remotely attracted to you, just say "let's keep going like this" or something to that affect, after she gives you head, or ask her to slide back and forth on it (nuru style), chances are VERY high that she'll let you slip it right in.
