My first and second visit I only paid to my provider.
My first visit (see my prior review) I asked provider 'how much'. Her answer was 'how much you pay' (or something like that). I said $160 (based on prior comments) and she said $180. I said that works and gave it to her.
My second trip (based on comments here) I went ahead and tipped $20 for my prior weeks playtime. (I felt bad & cheap) As she didn't 'get it' at first or second or third explanation it was a wasted bonus. I give a $160 to $180 up front at the start and they handle everything from there.

Sometimes they offer it up, sometimes you have to ask. I assume it is based on how much you paid up front.
