You will most likely be asked for references from one or more other providers that have seen you, if you cannot provide references they may ask for personal information.

They may if you are a member of TER and for your handle so they can look at your reviews and perhaps require that you write a review after the session.

Depends a lot on how legitimate the place is, be worried if they don't screen. If you really cannot provide references you will have a difficult time getting an appointment without divulging personal info.
