As a federal employee working for the NIH, she can legally practice medicine in New York State, or at any NIH or other federal facility she's appropriately credentialed with (NIH Bethesda, MD in her case) regardless of her state of licensure. Note "psychiatrist" = MD or DO plus psychiatry residency, state medical board licensed and has medication prescribing and admitting privileges. Psychiatrists like to medicate you out of your issues. "Psychologist" = PhD, no prescribing privileges without additional education / certification. Psychologists like to listen to you talk while you sit on a comfy couch. Both medication and couches have their place.

Maybe you are blind, but I have NEVER seen a homeless person with a gun at their head, a knife at their throat, or someone in a position of authority FORCING homelessness upon them, or addicting substances INTO THEM (please describe ONE INSTANCE where an inanimate substance FORCED ITSELF into ANY HUMAN BEING without the human being willingly, knowingly, and VOLUNTARILY acquiring and ingesting (and RE-acquiring and RE-ingesting!) the substance. You cannot say that substance addiction is like cancer, can you? It's not even like, say, bipolar disorder. The predisposition to becoming addicted might be an illness, but like eating pork when heart disease runs in the family, you have a choice to live a healthy lifestyle.
