If not using intake forms then likely other compliance issues exist resulting in and easy news story. I like to go for a massage at least once per week but am making sure everything is in line with requirements. There is still nothing wrong going anywhere for a massage. It seems even the well known specialized massage places have upped their compliance game.

I don't think I can post a link because it reveals names. The bill is in PDF format if you do a google search. It also appears on several law websites and is a good read. You can go back a few pages on this forum or search all FL forums. There is a lot of information out there with commentary. If you need a direct link to the bill you'll have to do it by PM. I'm not trying to be evasive, I'm just careful with what I post so it does not break the rules.
