IMHO, nothing happens without trust and (usually) an established relationship. If they're not comfortable working on you without draping, likely they are enforcing their own boundaries and / or their perception of the law in their state. Personally, I prefer no draping regardless of the extras and am often successful wherever I have gone. There are some excellent LMTs in the Boston area that will accommodate this preference.
Again: don't be an asshole and build a trusting relationship first, over time. If you get excellent massage work (and you often will), that comes first. Anything else is extra. If you don't care about a decent massage or want more of a guarantee, go to the bodyrub girls.

What about the lmt being married? Does that dramatically decrease the likelihood of extras, or does that not matter much?
GiVin out info on an LMT is dangerous for her and seeing as you're quite new here, some would say your question is suspect. Any LMT would not want to have guys from a sex board showing up at her place of business. In fact, a well known LMT / nurse (NL initials) is all I'll say, found out she was being discussed here and she was livid because creeps were showing up and she was approached by someone who needed a medical LMT and did a Google search and not only did her website come up, but so did the threads from this board. Needless to say, she could have gotten into some serious trouble if that potentail customer decided to report her. Keep the legit LMT's separate from others out of pure respect. I saw the fear in her eyes and it's something that could have been avoided.

I know the basic rules of how to go about it once there (no asking. Ever. No touching w / o permission. Etc), but are there signals I can give that I'd be willing to tip? I'm going to assume 0% chance on first visit anywhere, so perhaps pay in cash and give a $. 1 or $. 2 tip to show I'm a generous tipper for next time?

I've had the discussion about reviews with more than a few LMTs I know and, without exception, each has NOT wanted this kind of review. What Gallo1 says is not only courteous, but helps them avoid so many problems. Don't jeopardize your relationship with these professionals.

My limited sample set of nine (not in the Boston area) are distributed like this:
2 single, never married, no kids.
6 previously married, most with kids.
1 in committed lesbian relationship, previously married w / kids.
So, none were married at the time. Before you ask, the last one on the list was excellent: her deep tissue massage was top-notch and she had no problem with men (just her ex husband, LOL).
