This may be expensive if you have no insurance, but I can guarantee you that the cost of seeing a provider or clinic is far less than trying to get this monkey off of your back and going cold turkey. While NA doesn't provide rehab the people who have been there awhile generally know where to look for additional services.

MRT (methadone replacement therapies, which actually often use suboxone) are easier to sign up for but the problem with them is not the token amount they charge (effectively less than these girls get for just one BJ). The problem with them is 1) holding on to the money they need until its time to go in for their dose (usually first thing in the morning) and 2) waking up early enough and getting to the center in time so they can get in line and get their dose before the center stops administering their drugs for the day.
In many respects, the cold turkey during jail time approach can be more effective because it doesn't leave them any other choice (plus it doesn't replace one addiction for another) but it is a REALLY hard and painful way to get clean.
As for employment options after getting clean, Flav also summed it up pretty well. Basically either 1) returning (or continuing) to hooking or 2) some low paying menial job. Having a felony on their criminal record (as many of them have) certainly makes the latter work harder to find, though not impossible (unless their felony list is really long) and not just with mom & pops (I've known former addicts with records get jobs at 7-11 or Subway, though keeping those jobs was something else). The problem there is not simply their criminal history but their near complete lack of BASIC job skills (like waking up and showing up for work on time and not just when THEY feel like working, following up after a positive interview, having even a minimal work ethic, etc.). Its almost like they don't really want to work for minimum wage or a set schedule after they've become used to going out whenever they feel like it and simply standing around until some guy comes around and offers them $20 for 10 minutes work.
But the other alternative (returning to hooking after getting clean) is really so much worse. If they return to hanging out with the same old crew (drugged out fellow SW's, DB's and often even some johns who used to do drugs with them), what are their chances really of staying clean? If they were really any good you'd see many more former addicts working the streets, but you don't.

That's no joke. Can't tell you how many people I've seen who are in jail for something else and then end up coming to the hospital because of withdrawal symptoms.
It also speaks to what a piss-poor methodology we take to help people in drug rehabs in this country. Switzerland had a large problem with heroin overdoses and crime in the 80's and changed their methodology to harm reduction. Basically gave addicts free heroin, clean needles, medical care. But it also came with the stipulation that you had to stick with a social worker who would help you address the fundamental lack of jobs, housing, etc. Since then they've had a massive reduction in crime, ODs, and some 60-70% of the people in the program were able to kick their habits and maintain jobs as functioning members of society. It just goes to show that culturally we really don't do all we can for people who fall through the cracks in life here.
