I have purchased Walmart Visa debit gift cards and there is no monthly fee that is taken out. You have the option to register it at walmartgift dot com for online purchases (which require an address) and you can enter any address you want. Your name is not associated with it at all. These can only be used for purchases within the US. I used these in the past to order magic pills from JCM (they used to have a US payment processor) but they only take bitcoin now.

The Visa gift cards I was speaking of are the Green Dot brand. I'm sure An349 is correct about there being some cards that do not charge a monthly fee.
