In Oregon we have the State Medical Board to send complainants to. I suggest looking at the State Of Washington Medical boards for guidance.

They ask for your information, but you can just make it up. If you want to stay completely anonymous, I don't think they take cash, but they would likely accept a prepaid gift card which should do the trick. I've never been asked to show I'd, but if you're concerned about it just order a fake one from OIS.

I test out of a nearby Quest facility within a Safeway. The staff there has always been courtious, professional, and 90% on time.
