Minimum attire is t-shirt and shorts, with boxers not being considered as appropriate shorts. However, in practice, at least 80% of the girls will bend the rules. I've cuddled in underwear and I've cuddled naked more than once. Most of the time for me it starts in underwear and the undies come off after a few minutes.
My standard routine if the girl is hosting is to walk into her hosting site (usually a hotel room, but on rare occasion her home). Once I'm inside and we have the introductory hug out of the way (which is pretty typical), I'll say something like "oh crap, I left my cuddle shorts in the car. Should I go down and get them or can I just cuddle in my underwear?" I'll have a pair of thin nylon running shorts in my car just in case - the type with the mesh liner kind of like a swimsuit that doesn't need underwear. It may be luck of the draw but I've never once had a girl tell me to go get the shorts. The typical response is something along the lines of "whatever makes you comfortable. " If she says that it's a license to take my pants off, and I'll probably even take my shirt off at that point and be wearing only undies to start.
If you're hosting, the "I left my cuddle clothes in the car" trick doesn't work. In those cases I'm in a hotel room and I answer the door wearing a t-shirt and my thin running shorts. The cuddle will start from there. A lot of times the girl will bring cuddle clothes to change into. She'll go into the bathroom to change and there's a good chance she may come out wearing something sexy or skimpy. I've had girls come out wearing skimpy tops with no bra underneath and shorts with half of their ass hanging out. I've had girls come out wearing lingerie. I've also had girls come out wearing a t shirt and thick sweat pants, and I've had girls just take their shirt and pants off right there in my full view to start the cuddle in bra and panties. You get the idea - it's all over the place. But if she comes out wearing lingerie, or something skimpy, or undresses in front of me, rest assured I'm going to have the rest of those clothes on the floor pretty quickly.
2. YMMV in this area. Every girl is different. I'd say the most typical scenario is you start cuddling and making close contact. Your arms around each other or maybe you're spooning. Once you're comfortable, let your hands wander OTC a little. By this I don't mean go grabbing body parts. Just sort of gently graze OTC. If she's a by the rules girl she will quickly stop you and ask that you don't touch her there. If she doesn't brush off some light OTC touches you can get a little more assertive by going up the shirt and / or down the pants. If you're able to touch any intimate body parts under the clothes it's game on and things will usually escalate pretty quickly and naturally from there.
Bottom line: Be patient. Be a gentleman. Don't be creepy. Don't get grabby or pushy. Expect nothing but a nice platonic cuddle with a nice girl. Be elated if clothes come off. Be prepared for a strictly platonic session once in a while, and when one happens just enjoy the hour in bed with a nice girl, even though it's platonic. Chalk it up and move on to the next one.

If you play your cards right you can get your nut busted 75% of the time. That's about what my success rate is. There are girls on there who have pressed sexual assault charges on guys who crossed the line, so you do have to be careful. But when extras are going to happen it's usually intuitively obvious that they're going to happen and clothes start falling off, very much like they would on a normal date. The thing is there are very few hidden clues, and the ones you read into profiles aren't always accurate.
Most girls say "platonic only. " Don't take it at face value. I've had some pretty steamy sex with "platonic only" girls and have been blue balled by ones who didn't say that.
Sexy pics. A lot of focus on butt, cleavage, skimpy outfits etc are USUALLY a good sign, but not always. Some of the highest mileage sessions I’ve had have been with girls who had very PG looking profiles. Some of the worst sessions I’ve had have been with girls who had pics of themselves in lingerie or skimpy bikinis.
Few of these girls are sure bets. Strange as it sounds, that's what makes cuddling interesting. Most of us don't want sure bets. We want the girl next door who doesn't spread her legs for 10 guys a day for a living. If we want a sure bet we can go to an AMP and get laid anytime we want very easily. Most of us are into cuddling because we got tired of the AMP scene. The thrll of the chase brings a fun and interesting new spin into the hobby.
The best thing is trust your gut. If you see a few you like, try to get some intel. Ask around on here (preferably by DM to some of the mongers who have experience with cuddling so that we don't publicly out the girls too much). Understand that even with intel it's usually not a sure bet, but you can increase your odds substantially. I’ve gotten extras and even FS from girls who aren’t widely known to provide extras. I’ve gotten nothing from girls who are well known extras providers. A lot about cuddling is how well you vibe with the girl.
Always look at it this way. You’re in a private room with a girl, in a bed (usually), quite often minimally clothed, all while making close body contact. You have your arms around each other. You’re spooning. Hands naturally end up on each other’s bodies. All of the ingredients are there for anything to happen, even with a girl who doesn’t include extras as part of her routine. If she likes you it doesn’t take a huge stretch of the imagination to see how clothes can start falling off.

They're not the majority by any means but if you reverse search their profile pics some of them will lead you right to their escort ads. That's not what the majority of us who use cuddles want. If we want a sure thing we'll go to an AMP or hire an escort.
Upsells are all over the board. Some will do more without asking for more. Some will flat out give you a menu of services with prices once you're with them. Some will ask for a paid second hour. A small handful have even been known to offer freebies on a second date if they like you. There really is no standard.

CC has forums where the admins post and they have discussed this website. Their position is that if someone posts on here that "MsXYZ gave me a HJ" that is not evidence of a rule violation because the poster could be lying.
People usually get banned for trying to skirt fees, giving out contact info without a confirmed session, manipulating the fees / hours etc. Of course if they messaged on CC that they might give out an HJ, that is a different story. But these are all things the admins there can verify on their site.
Granted if I went on this site and posted "MsXYZ gave me a HJ", then the CC admins might look at her account for some other rule violation.
And of course with CC, with a lot of the ladies your mileage may vary depending if they like you / feel comfortable with you / are having a good day / etc, so posting you got a HJ from someone isn't a guarantee that the next guy is going to get one. I have a lot of ladies from the site text me before a session reminding me to shower and wear clean clothes. Seems like a no-brainer but they wouldn't be telling me that unless there be some nasty dudes on the site!

How do they get proof that a cuddler did a session naked and / or provided extras? That's extremely difficult to do. They could directly message you and ask if the cuddler got naked for the session, but even if you said yes her clothes went flying across the room 30 seconds after she walked in the door, and a minute later my dick was in her mouth, that's still not proof. I suppose if the mods messaged half a dozen different guys and they all openly admitted to her providing extras then they might have reason to ban her, but that seems pretty unlikely.
