Either a number for the therapist or the establishment, with the State of Florida Massage board.

Saw few reports here so decided to give this one a try since it's so close. 50 for hh. When I was finally on table I noticed it was absolutely freezing. I felt like more of a doctors office. When you go in and tell them this, they usually don't charge me an Extra $5 Fee.

From what I learned, charging you more is not illegal. It is illegal to not tell you there is a fee but charge it in the State of Florida. It was never illegal but it used to be a violation on the terms of service for Visa / MC / Amex to charge a fee but that is gone now. My apartment complex 10 years ago started charging a fee to pay by CC and I dug into it deep then. FYI, the clerk of courts in my county charge extra % to pay property tax on a CC. Gas stations no longer have to say Cash Discount for the cash price and you will notice many don't anymore.
