I went to an out-of-network / specialist doctor just a month ago, where I was in exam room for 1 hr 15 min, a full 55 minutes with the doctor and a nurse, and about 15 min with another nurse. I was charged $275 (including tests), which is approx $220 / hour.

Like BC, I am older, and more established. That comes with higher expectations than stiff-dick 20 and 30 somethings when it comes to satisfaction.
