- Business Name (as listed in online advertising or web search).
- Location / city.
- Name of provider (please always ask for good / bad report or so you know who to ask for next time).

- fee plus tip.
Without this information you will either be bombarded with pm's or a slew of unnecessary follow up questions in the forum.
