Any time I have been there, they have not requested the house fee upon entering. I just placed $200 on the table, get undressed, lay on the table and wait for them to come in.

I think the reason they want the last 4 digits of your phone number is so they can look up what you paid total the first time you were there and that's the amount they will want from you.
