QV only refers to the time spent with the provider, and everything is on the table. Although, I've read many times that some will use qv to get you in the door, and then up-charge you. I avoid these woman like the plague.

Indeed, with a QV, you'd expect those aspects to appear more rapidly than with a longer visit as you don't have time to hang around! A provider imposing restriction during a QV should, IMHO, be reported and shunned.
