The VIP room is not exactly conducive to extras given the curtain only comes down halfway from the top of the ceiling. I have been offered a HJ from another girl on nights once, but I never asked any girl for extras as I really didn't know if it even happened here. But then again given it pretty much happens in the open at Bogarts for all the world to see who knows.

There are much better / more consistent options in Detroit. Try BT's, Coliseum, Hustler, or Penthouse before John's Hot Spot. Google for locations, talk to the ladies and the option will be there for you leave a happy man.
