Not specific diagnosis per say but a quick Google search can state what it's probably for.

No insurance needed. Most medications are even cheaper without Insurance!

I went to the health department for the first time a year and a half ago. I was concerned at first but they said they have to keep my results confidential. It was free and I've been there three times since with a clean bill of health each time (knock on Wood). They provide free condoms (a bag of 10). They have never contacted me by mail or phone. To They give you a phone number to call a few days after the test. Just my experience and I hope this helps.
Definitely use the health department. It is free, confidential, no call from them. Everything is done with a code they give you.
Also, should you end up with the dreaded STD, they will help you with medications as well as with going through your legal obligations and helping you with that.

Best practice is to start an antibiotic immediately, rather than wait for a culture. If hearing a patient's concerns about unprotected sex, experience may lead a doctor to be proactive about a comitant asymptomatic case of gonorrhea or chlamydia. Properly dosed for age and weight, using CIPRO is likely best practice in THIS MAN'S CASE. YMMV.

That's why it's more common in females.
