I don't want to go into details of cash or performance with any particular lady, both because I don't want to violate anybody's confidence and because the details aren't set in stone (or even a fixed menu), so experiences may vary depending on all sorts of hard to predict factors like the model, the client, and how well they happen to click that particular day for no particular reason.
Still, I hope it is helpful that I can say after many visits at ST:
1. The posted rates include only what in this forum would be called a nude standard NoVa finish.
2. For that service, additional tips are not expected but appreciated.
3. I tend to tip pretty generously particularly at a place like ST where I expect to return often.
4. Additional services may be volunteered, in which case I think a generous tip is expected, or negotiated for an extra tip.
5. The tip (expected or negotiated) for any additional services short of FS tends to be about the same as my standard tip, so it feels like they are free when offered.
As for the specific dream of Samantha, while I can say that it lasted only half an hour, the total cost was about half of what I usually dream of spending on an hour with a quality lady of negotiable affection and left me equally satisfied.

I understand that this is frustrating if you are looking for one particular thing and you cannot know if you'll get it before committing. But even if something was offered 90% of the time, I wouldn't want to say so lest somebody in the 10% is disappointed and then accuses me of being a liar or a shill.
One piece of advice I can give:
Don't assume or rely on vague feelings or just "try" things. These women are human beings and trying to slip something by them, so to speak, may very well offend them, as it likely would you or me.
But on the other hand, none of the women working at establishments where there is any erotic contact are virgins fresh out of the convent who are going to run from the room shrieking "pervert!" in response to any polite request you can think of. They have most likely heard about, seen, been asked for, or even done things freakier than anything you (or at least I) have.
So once you are in private and have built a little report, just ask for anything you'd like to do. Quite often the answer will be "Yes" or "Yes, but I usually get an extra tip for that. ".
Worst case is "I'm sorry, but we are not allowed to do that here. " or "No, I only do that with my boyfriend. " or "I can't because I'm on my period. " Even if she thinks "Eww, you / it are way too dirty / old / fat / gross," she is almost certainly going to decline more politely. And in that case, just enjoy what is available with good grace and no pestering. But feel free to ask again once every time you visit.
She is very pretty and knows it. Massage is average but relaxing and the flip is slow and happy. There Are no extras beyond the standard nova. My opinion is she is decent value but I would go to a Thai indie for the better massage and service.

It is quite reasonable with $60 being a one hour massage, $70 with the table shower. If you buy a: gift Certificate" It is only $100 for Two one hour massages. So for 50+10 + a tip you get a good massage and table shower. I typically tip $20 so a total out the door of $80. Hard to beat.
IME there is no chance of any extra here.

I don't really care for the mentality that just because you received some sort of service than you should tip. To me tipping should be used as a way to reward good behavior or a way to communicate your satisfaction with a certain level of service. I don't necessary agree with always tipping no matter what. I think we should be free to use your discretion on if and when we tip. Just my two cents.

Over the last few years my experience is no extras but one particular provider offers to squirt lotion in your hand and then starts rubbing your balls for an assisted DIY.

If I understand your post correctly, you didn't actually see the provider. If that's the case, then your comment about "better / safer" options is more about your experience with the person doing the texting, rather than a reflection on the masseuse herself or her services.
Over the years I've probably seen most of the Thai indies and, while there's a lot of variation among them, one thing that's pretty constant is a high level of cautiousness. If you think about it, most have a lot to lose, probably more than non-foreign providers. Needing to support family back in Thailand while avoiding LE and immigration issues here (except for those who are citizens) would make anyone hyper-cautious. Also, some of the providers I've befriended have shown me text messages sent by potential clients. If I hadn't seen it with my own eyes, I might've doubted them. But guys who aren't known to the providers come right out of the gate asking for FS, GFE, Greek, etc. According to the Thai indies I've talked to, those guys get blocked, labeled as possible LE, and their numbers are shared with other providers.
As I said, YMMV, but I've personally never had a bad session with a Thai indie provider. I've had some average ones, but those have been far surpassed by the exceptional. I obviously wasn't there for your text interaction, but you might want to look at it again to see if anything about it might have triggered those defense mechanisms. Of course, the other possibility is that you just caught her on a bad day at the office. Maybe just before you texted she was dealing with some unknown jerk trying to get her to commit to FS via text. Or maybe she just had a no-show time-waster. Again, I have no idea and I wasn't there, but my personal experience has been that respectful perseverance with the Thai indies has yielded great rewards.
