They see the VPN IP and say "This guy shouldn't be connecting from 'insert city'. ".

I would also check Blink Health as they might have a better deal based on your location and the stores near you. I can remember the days of paying a small fortune for the brand name drug and splitting a 100 mg pill. Luckily I was seeing a reliable 21 year old SB so it never went to waste.
