I had planned to leave the biz a year or so ago, more recently, I stated that I was only available for regular clientele, or those who don't mind a little screening. She had a similar form on her website that asked for all sorts of crazy shit; full name, home address, employment info, position at work, an "official story" to tell whomever answers the phone when she calls you at work etc. I bypassed the entire form and simply emailed her respectfully requesting an appointment while explaining my privacy concerns. I gave her my date check handle along with 2 references and she agreed to see me.
I think the uber militant screeners instinctively know some of the things they ask for are insane and will capitulate if you can assure them that you're safe without giving them your mother's maiden name or whatever other malarkey they ask for. Is any sane business woman going to let a client's refusal to show his driver's license or reveal his last name stand between her and a big fat bouquet full of hundreds of roses, provided he's already given a reasonable amount of info?
