You sure are a great guy and I bet your providers love you but I agree with Scohen everything should be in the requested donation and if you want to tip maybe a little extra but not 50 and up.

Just Tip with what you feel comfortable with. I'm not saying $50 is what you should give. But that's at least what I give to my Provider. I figure that a minimum of $50 for what she does for me is a good tip. But if she's cool with her Donation price with Extras included, then all well and good. If you do leave a Tip make it at least a fair amount for what she does for you.
