This isn't a secret template or formula. You will need to adapt on the fly based on how they answer and how eager they seem. I've lost out on good POTs because I spent too much time in the flirty / small talk phase, and I've lost POTs because I moved too quickly to discussing arrangement details or sex. You're going to fuck up and lose some.
