The big part is it adds so many layers of complexity that it isn't simply fixed with a pill. It damages organs, leads to early dementia, ever time you get any infection it's like complicated. Genitalia lesions, pneumonia, pyelonephritis, sepsis, meningitis, etc. Every kind of infection has to have infectious disease consulted. In general, these people are much more sick and docs have to take drastic measures to save what's left. But each time, bit by bit, worsening mortality. They process of dying is severe suffering, a quality of life nobody in their right mind would ever want to chance. It's horrible what these patients go through with endless hospitalizations. They get resistant strains of bacterial infections, have to be on chronic antiviral and antibacterial medications that will lead to C. Diff infections and blow their colon out. Everyone has this idea that modern medicine is way more powerful than it really is. We haven't come far from the penicillin days, maybe a cure for hep c. But by and large it's the same stuff that's been out since the 80's with AI now. Once they get to a certain CD4 count, they become walking petri dishes and super spreaders, it's terrible. If you have Prep and take it regularly (anyone bareback needs to be on Prep medication) you will have a minimal chance of getting the virus, and prophylactic doxycycline for to prevent GC will work for most.
