If I'm wrong, I'm wrong. I'm just going by what State DHS 145.04 (2)(A) says the report must contain:
The name of the ill or affected individual, that individual's address and telephone number, age or date of birth, race and ethnicity, sex, county of residence, date of onset of the disease, name of parent or guardian if a minor, and other facts the department or local health officer requires for the purposes of surveillance, control and prevention of communicable disease.
If that's just for statistical analysis, great. Here's to hoping none of us even need to visit the Dr for that reason.

Long time lurker, long time monger, first time poster. Noticed the recent trend to combine animals and letters, decided to join the bandwagon. Saving now for a attention getting Sports Car to impress the SW's and Dancers with. Spending lots of time upping my skills at Blackjack so I can win at the casino and afford top end escorts.

Thank you to everyone that has TOFTT, I expect nothing from anyone until I prove myself on the board. To that end I have a semi-utr that lives in an Apartment near the airport. Had senior status but I think I had too long of a lapse in posts so they were deleted; no clue and doesn't matter! But I disagree with you in regards to his input. 19 has given input but considering his mental state I wouldn't trust anything he writes on here. He clearly has mental issues and if you don't realize that than you might want to check yourself out. His input might be true or spot on but I would never consider it reliable, ever.
