(therefore the ubiquitous YMMV) I would not insist on taking it off during the date. IMO that would be rude. These ladies are human beings. Respect their wishes. Try asking nicely instead. And respect their answer.
With M there is not a standard upcharge. So it is 260 or 270 or whatever. Most of the ladies do not collect the money first. I have seen a number of ladies multiple times and they still offer the same service whether you tip or not. I think tipping is over-rated. They just see you as a generous ATM, or more likely an easy mark, but they won't change their menu the next time you see them just because you didn't tip the last time. The key is to be nice.

They tell you whatever you want to hear then when you get there the girl says"it's her friend you texted and she doesn't do that. It's CFS but you are there so you indulge 8 out of 10 times. It's called the "bait and switch". I have on occasion seen a cute girl there but that is really rare.
