There are a couple of ways you could play this from here.
1) You build a cover story asap and get out ahead of it. Create a narrative that the scammer has stolen your identity and is using it to try to frame you for having an affair in order to extort you. If they have incriminating evidence on you, then come up with plausible reasons for how it could have been fabricated to make you look guilty. Once you have your cover story ready, then you go to the old lady and tell her you've been hacked / extorted / etc and find out if anyone has contacted her.
2) Tell the scammer to fuck off. Keep in mind that what the scammer really wants is money. They don't really care about ruining your life, they're just trying to scare you enough to get you to pay them. If you tell them to fuck off, then chances are they will leave you alone and move on to another mark without bothering to contact your wife. Or if they contact her, you can just blow it off like any other of the million bullshit scams out there nowadays and don't make a big deal out of it.
Of course each of these approaches has risks and could backfire. #1 might cause your wife to get suspicious and / or not believe your story, since it's true that you are fucking around. And #2 might cause the scammer to say fuck it and contact your wife anyway, which could make for a very damning first impression. For me it would depend on how much incriminating information the scammer has (ie pictures, videos, messages, etc) and how confident I was in my ability to lie to my wife and convince her I hadn't done anything wrong.

P.S. It's a good reminder of why you should always, Always look after your privacy when dealing with sex workers. Yes, even with sugar babies. Never let them have your real phone number, address, private photos of you, etc especially if you are married / attached and need to be discreet.
