EVERYTHING in that shot was staged by actors, extras, etc. In order to splice once take with another EVERYTHING in the background needs to be perfectly consistent. You've got people on set whose only job is to make sure the props, costumes, set, make up, lighting and EVERYTHING is consistent from take to take. Unless it is reality TV, it is likely staged, no matter how raw it is shot to make it look like reality.
